Parent sign-up slip card — worked example
Last updated: October 11, 2026
Maple Street Math Club sends one term sheet home per student. The old way was a paper stub that lived in a backpack; this card holds the whole reply: name, session yes/no, and the allergy line the teacher actually needs before Saturday.

Submissions land on the data page as they arrive — with the state the card promised: spots left, full, or waitlist.
One question this sample gets asked
Why would a school flyer carry a card instead of a QR to a form?
Because the state comes back: who is in, how many left, and the form closes itself at the room size. A plain QR to a bare form keeps collecting names for a room that is already full.
