Join-and-vote card — worked example
Last updated: October 11, 2026
Mudlark Books runs a Thursday night club. The card on the counter does two jobs at once: join the club (name and the email the notes go to) and vote for next month's title from four candidates. The vote counts ride the same live list as the sign-ups.

Can one card really run two questions?
A card carries the fields you set — this one takes an email and a choice. What it deliberately doesn't do is multi-page questionnaires; that is FormLM's territory, and the guides link from this page says when to switch.
